After you submit the pre-registration form, links to additional forms will appear for you to complete.

Please print and complete the registration paperwork.
When registered, print and complete:
- Child/Adolescent Intake Form
- Adult Intake Form
To authorize us to communicate with any current provider(s) you are working with, complete a Revised ROI for each provider.
If you plan to enroll your child in the social skills group, complete the Social Skills form.
